Certificate of Naturalization (CON) – Lost or Missing

Replacement of a Lost or Missing Certificate of Naturalization

Required Documentation

To proceed with a replacement request, the following documents are required:

  1. A certified copy of the Certificate of Naturalization, if available
  2. Two (2) certified passport-sized photographs
  3. A Statutory Declaration detailing how and when the Certificate was lost
  4. A Police Report confirming the loss or disappearance of the Certificate
  5. A certified copy of the applicant’s Dominican passport
  6. Submission of the application with a cover letter issued by an authorized agent

All documents must be clear, properly certified, and submitted in accordance with official requirements.


Amendment of an Existing Certificate of Naturalization

Where a Certificate of Naturalization contains incorrect information, an amendment may be requested. This process requires the return of the original Certificate and additional supporting documentation.

Required Documentation for Amendments

Applicants must submit:

  1. The original Certificate of Naturalization containing the error
  2. For name amendments:
    • A certified copy of the applicant’s birth certificate from the country of origin
    • Subject to due diligence verification
  3. For address amendments:
    • Proof of address, subject to due diligence verification
  4. An Affidavit explaining the nature of the error on the Certificate
  5. Where applicable, an Affidavit from the translator if the error resulted from a translation issue
